International travel is an exciting adventure but also poses many unpredictable situations. One
of the most serious is the risk of injury or illness in a foreign country. Unfamiliarity with a different culture and medical
system is intensified without the proper resources to assist you in securing medical care and covering the associated costs. While
traditional medical plans offer fine domestic coverage, few are designed for international use or visitors. Even those that offer
limited benefits do not normally cover medically supervised emergency evacuations, emergency reunions or repatriation. Additional
complications are caused by different languages, currency conversion, inexperienced international claims administration, and time
differences. Imagine trying to get in touch with most insurance companies at 3:00 a.m. on a weekend - from a foreign country -
during a medical emergency!

making international medical coverage convenient and affordable. There are three different Patriot Travel Medical
Insurance® plans available. At enrollment you simply pick the plan that applies to your citizenship and travel
plans. Patriot Travel Medical Insurance provides up to US$2,000,000* of medical expense coverage, up to US$25,000 in Accidental
Death and Dismemberment coverage, a complete assistance package of international benefits and 24 hour availability for your total
peace of mind.

Optional Policy Riders
Leisure Sports Rider
The Leisure Sports Rider is available on both Patriot plans and adds coverage for jet skiing, snow skiing, snowmobiling, snorkeling, surfing, wake boarding, water skiing and windsurfing. For premium information, please see the back of the Application
form.
Extreme Sports Rider
The Extreme Sports Rider is available on both Patriot plans and adds up to 12 months of coverage for those under the age of 50. This rider provides up to US$50,000 lifetime maximum for the following activities: abseiling, BMX, bungee jumping, canyoning, caving, flying (private plane), hang gliding, heli-skiing, high diving, hot air ballooning, kayaking, inline skating, mountain biking, mountaineering (ropes and guides to 4500m from ground level), paragliding, parascending, rappelling, rock climbing (ropes and guides to 4500m from ground level), skydiving, spelunking, whitewater rafting (to Class V). These extreme activities must be engaged in solely for leisure, recreation, or entertainment purposes.
The monthly Rate for the Extreme Rider is available in monthly increments up to a
maximum of 12 months. For U.S. Citizens the monthly cost is US$30.00, Non-US Citizens is US$45.00.
Patriot Return Rider
The Patriot Return Rider is only available to non-US citizens who have purchased Patriot America . When purchased
at the time of application, Patriot Return provides temporary medical coverage for non-US citizens returning to their home country.
The insured person must be outside his/her home country at time of application.
The Patriot Return Rider cost is calculated by multiplying the
factor of 1.05 by the total premium. Please see the premium calculation section of the Application
Form.
Patriot T.R.I.P. Lite SM
Trip cancellation coverage is also available for your trip. With this plan, the loss of pre-paid,
non-refundable, and unused payments may be recovered when a trip is cancelled or interrupted due to:
illness, injury or death to you, a
family member or travel companion; bankruptcy or financial default; a terrorist incident; jury duty; home made
uninhabitable by fire, wind,
storm, flood or vandalism;quarantine;auto accident on way to airport; job termination; cancelled leave for active duty military, police
or fire fighters. The benefits are outlined below:
Benefits: Maximum benefit per insured person
Trip cancellation for Trip cost up to US$20,000
Travel Delay up to US$500
Baggage delay up to US$100
The Patriot T.R.I.P. Lite cost is calculated by dividing the trip cost by 100 and multiplying the result by 4.52.
Example: A Trip cost of $2000.00 divided by 100 = 20 X 4.52= $90.40

Enhanced AD&D Rider
This optional coverage is available for the primary insured person only.
This coverage is in addition to the Accidental Death and Dismemberment
already included in the Patriot plans. Coverage must be purchased for
a minimum of three months.
